- purpose of the Program
- who is eligible to enroll and how to enroll
- the costs associated with the program
- how a continuous glucose monitor works
- what services are offered within the Program
PURPOSE OF THIS SESSION: Clarify how the Program works and what services are offered.
FORMAT:
- Half hour zoom-based session
- 15 minutes for a presentation followed by 15 minutes for questions & answers (English)
